The 72-hour window
Urgent: If shingles affects your eye, ear or face, if you have a weakened immune system, or if symptoms are severe, seek medical care promptly. Antiviral treatment works best within 72 hours of the rash appearing.
Early warning signs
Shingles often announces itself before the rash appears. Recognizing these early signs can make a critical difference — treatment started within 72 hours of rash onset significantly reduces complication risk.
Common early symptoms include:
- Burning, tingling, or shooting pain in a localized area on one side of the body
- Itching or numbness in the same area
- Skin sensitivity — even light touch may feel painful
- Flu-like symptoms — fatigue, headache, mild fever
The 72-hour window
Antiviral medication (aciclovir, valaciclovir, or famciclovir) is most effective when started within 72 hours of the rash first appearing. Starting early can:
- Reduce the severity and duration of the outbreak
- Lower the risk of postherpetic neuralgia (PHN)
- Reduce viral shedding
If you suspect shingles, don't wait to see if it gets worse. Contact a healthcare professional as soon as possible — ideally the same day the rash appears.
When the 72-hour window has passed
Antivirals may still be prescribed after 72 hours in certain situations, such as:
- New blisters are still forming
- Eye involvement (zoster ophthalmicus)
- The person is immunocompromised
- Severe pain or widespread rash
